The  final balance due and final chair cover count is due 15 days before the event.

Shipping
Shipping cost is estimated using UPS Shipping Internet rates, round trip shipping,
guaranteed delivery and is paid by debit/credit card the week before the event. Initial_______

Chair Cover Return
The chair covers are returned on the Monday after the event. Please call UPS Shipping for a
pick up after applying the return labels and tape or drop the boxes off at a UPS Store in your
neighborhood.  A late fee of one half the rental cost is imposed if the chair covers are not
returned to UPS Shipping on the Monday after the event. Please initial__________


In the case of an Act of God weather event preventing delivery to your address or
cancellation 30 days prior to an event, all monies paid are non-refundable. Any delay in the
above process will result in a fee of $25.00, additional shipping fees and/or cancellation
without refund. All changes to your linen account/rental order form, must be made by email.
